To do that, get toCradle of Sulevin.
After you get there(M26,1a), you will have to gather fourfragments of the sword.
You find them near the altars(M26,1b-1e)in the center of the rooms that you enter.
After you activate each one of the altars, you will be attacked by a Revenant.
Only after you kill him and loot him, do you find asword fragmenton him.
Then, al of the remaining torches burst into flames.
After you collect all of the fragments, go toSkyhold, talk toDagnain the Undercroft.
